The goal is to make you feel happy, healthy and then get on with enjoying life. A healthy lifestyle can be achieved by having a healthy relationship with food. Life is too short to be feeling miserable, guilty, obsessive, tired and stressed about your eating style, weight and overall health and well-being.
This is you first consultation when your GP has referred you as part of a ‘Team Care Arrangement’ (TCA). These sessions are bulkbilled here so you pay $0. You must have a Dietitian referral as part of your TCA to be bulkbilled. If you don’t have a TCA then you may be eligible for one with your GP. You normally need to book an extended consultation with your GP to do this. If you are not on a TCA you can still see the Dietitian as a fee paying client without any referral required. See below.
This is a subsequent consultation as part of your ‘Team Care Arrangement’ (TCA). These sessions are bulkbilled here so you pay $0. You must have a Dietitian referral from your GP as part of your TCA to be bulkbilled. This enables you to have 5 Allied Health visits each year. You and your GP decide whether you want 1 to 5 Dietitian visits. If you’ve used up all of your visits on your TCA you can still see the Dietitian as a fee paying client. See ‘Ongoing sessions to keep you on track’ below.
If you have a DVA Gold card then your GP can refer you to receive Dietitian consultations free of charge. If you prefer home visits, this can also be provided for free. Eligible DVA clients may also receive heavily subsidised nutritional supplements (e.g. protein powder, protein drinks) if you are experiencing weight loss, reduced strength, etc. You must have a Dietitian referral under your DVA Gold card to receive these services.
This is your first consultation with the Dietitian when you’re a fee paying client. It's when you are referring yourself or your GP has referred you but you don't have a Medicare 'Team Care Arrangement' care plan in place. Private health insurance rebates may apply for you. $10 discount for Pensioners and Health Care card holders.
These are 30 minute sessions to support you to keep on track with your nutrition goals. You can also use these sessions if you've used up all of the visits under your Team Care Arrangement with your GP but you’d still like ongoing support. Private health insurance rebates may apply for you.
If you have issues with mobility or transport, I can usually arrange for home visits at no extra charge.
I can provide individual nutritional assessments and care plans for residents with a variety of health issues. Most consultations tend to be for those experiencing unintentional weight loss. Nutrition intervention for these residents can mean improved muscle strength, less fatigue, improved immune function, decreased risk of infection and occurrence of pressure sores, improved wound healing, less occurrence of falls and better recovery from illness or injury.
A menu review contributes to the Australian Aged Care Quality Agency’s accreditation standard ‘2.10 Nutrition and Hydration’. It assesses the adequacy of the menu to meet the nutrition and hydration needs of residents. It utilises current menu planning standards for Residential Aged Care Facilities, namely the Australian Guide to Healthy Eating, the Menu Audit Tool for Aged Care Homes (Dietitians Association of Australia) and the Best Practice Food and Nutrition Manual for Aged Care Facilities
Nutrition is essential for a resident’s physical and mental health, recovery and rehabilitation. I can train Nurses, Personal Care Workers and Catering staff to improve the nutritional health and wellbeing of residents. Education can be tailored to meet the needs of a facility. It typically includes nutrition screening, prevention and treatment of malnutrition and meal/drink/snack fortification, falls prevention, wound healing and special diets
Nutrition has a major role in meeting your physical and functional needs and contribute significantly to improved quality of life. If you have lost weight and muscle strength, I can provide nutrition advice to improve your health and well-being and to help you to have the best chance of maintaining independence in your own home.
